The COVID-19 pandemic has created unprecedented challenges for healthcare providers around the globe. In the UK, the National Health Service (NHS) tasked providers with identifying their highest-risk patients to focus resources where they were needed most. One organization, South-Central & West Commissioning Support Unit (SCW), leveraged the ACG System along with their own in-house expertise to quickly zero-in on the information they needed in a detailed, clinically appropriate way.
How did they do it? The SCW team cross-referenced ACG System data against specific patient vulnerabilities (physical conditions, mental health history, and social/environmental risk factors), dividing the information into three patient age groups. The result? A simple segmentation with an array of information that allowed SCW’s customers to laser-focus on specific subsets of patients. The customers –primary care providers – could slice and dice the information in multiple ways, specifically, to target older adults who had any two of the above listed vulnerabilities.
Ultimately, the ACG System helped SCW and their customers tackle the overwhelming challenges of COVID-19 by breaking the general population of at-risk patients into more manageable groups that could be prioritized accordingly. With this detailed information in hand, primary care providers were able to provide tailor-made support for the specific groups of people that needed it most, upholding their commitment to continuity of care and mitigation of risk for their patients.
