When people are released from incarceration, they often return to their communities with very little support. While incarcerated, the facility takes care of their health care needs, including maintaining health insurance, scheduling doctor’s appointments and keeping up with medications. After release, these responsibilities fall to the individual.
As a result, most returning citizens are likely to use the emergency department for primary care once they re-enter their community. These patients, many with chronic conditions such as high blood pressure, diabetes, heart or kidney problems and compounded with mental health challenges and unique social needs, often become an inpatient admission, leading to unnecessary costs and a lack of focus on preventative care.

This risk score was correlated to a resource utilization band (RUB) to predict the intensity of future care management needs.
