Case Management and Care Coordination Part 1

This webinar will provide an overview of the data available within the ACG System to support case finding activities. The topics covered will include the search criteria for nationally mandated programs such as the unplanned admissions directly enhanced service (DES) but will also cover examples of the other search criteria GP Practices and primary care based services are using to find cohorts of patients suitable for different types of care programs.
