Inter-Relationship Between Frailty & Multimorbidity

There is a great deal of focus on the issue of frailty in the English National Health Service. Whilst it is true that resource use increases with an increasing degree of frailty, this presentation will demonstrate that not all frail patients are high cost and not all high cost patients are frail. The importance of this analysis is that it highlights the need to:

• Undertake a thorough analysis of population health needs to understand the relationships between different segments of the population and to identify the characteristics of segments that are related to high cost and high resource use
• Choose the right algorithm or case finding technique to identify the segment of the population that you want to make an impact upon
