Inter-Relationship Between Frailty & Multimorbidity in East Berkshire, UK
Alan Thompson, MSc
Johns Hopkins Healthcare Solutions
Alan joined the ACG System Team in April 2013. In his role as Director of User Support, Alan’s primary responsibilities include supporting users, collaborators, and team members in maximizing the use of the ACG System. He also coordinates the UK Team in facilitating applications of the ACG System in the UK NHS. A registered nurse by background, Alan has over 30 years’ experience in the UK National Health Service (NHS) in clinical, operational and project management posts within hospitals and primary care. He has a keen interest in the use of information to support managerial and clinical decision-making and to inform improvements in the provision of patient-centred services. Between 2010 and 2013, Alan led a project to implement the ACG System across 600 GP Practices in the south of England. This role supported clinicians in optimizing the use of risk stratification information to inform clinical programs such as case management and integrated care teams. He also worked with Clinical Commissioning Groups (CCGs) to use risk stratification to support commissioning activities. As a firm believer in sharing experience and expertise across the NHS, he established both the Southern England and national UK ACG System User groups to facilitate conversations and shared learning among NHS users of the ACG System.
Sangeeta Saran
NHS Slough Clinical Commissioning Group
Sangeeta trained as a Pharmacist and completed MSc clinical Pharmacy in 2002. She worked in community pharmacy before taking up a role with PCT to deliver pharmaceutical advice to practices and community services. In this role she changed the delivery and focus of pharmaceutical advice and created a greater ownership at practice level of prescribing decisions and consideration of application of evidence base to make prescribing decisions at a patient and population level.
She then moved into a commissioning role as Lead for practice Based commissioning across East Berkshire. Sangeeta then moved into an Operations role, heading up Slough CCG since its inception. Slough CCG built a reputation of being innovative commissioners and was the first CCG to set up extended GP service at evening and weekends as the first wave Prime Ministers Challenge Fund.
Sangeeta now works as an associate director for planned care across Berkshire East and is operational lead for Slough CCG. In her current role she leads on CVD, Diabetes, Cancer and Neurology commissioning. Additionally, she leads on improving commissioning on pathways in planned care including referral management and adoption of e-referral in primary care.
