ACG GeoHealth: Using Social Determinants of Health to Drive a Successful Population Health Strategy

Social Determinants of Health (SDoH) have long been discussed as critical for understanding the social, geographical, environmental and economic factors which determine a population’s overall health and well-being. SDoH enable organizations to craft an intelligent and accurate population health strategy and can be pivotal in unlocking areas for better patient care and cost savings opportunities. For more information on SDoH, read our previous blog post here.

Building on the ACG System’s whole-person approach to health needs and predictive measures, we are excited to announce a new, optional component of the ACG System – ACG GeoHealth. This feature allows System users to utilize geographically relevant SDoH within their organization’s population health strategy.

What benefits does ACG GeoHealth provide?

ACG GeoHealth looks at where a population lives and helps users determine the risks and health challenges the population faces due to that geography.

Users can also determine how need-based plans, resource allocation and other strategic investments can meet the needs of different communities and develop plans to provide targeted care to a specific area. Utilizing these findings to focus on a population’s health risks will ultimately create opportunities to give all people in that area a chance at a healthier life.

What measures are available within ACG GeoHealth?

ACG GeoHealth includes 17 unique variables that measure health risks within a population. These measurements include poverty levels, home value, the likelihood of having health insurance and single heads of household, among others. When pulled together with the rest of the data available through the ACG System, organizations can create a full view of patient-level risks.

This information is critical when applying funding decisions and tactical support planning to a community in need.

How ACG GeoHealth Improves Your Strategy

A plan to improve population health in a specific geographic area is only as good as the data used to create it. ACG GeoHealth uses high quality, geo-targeted measurements to help you develop strategies to create better health outcomes and deliver more cost-effective care for your patient population. Having a full understanding of the SDoH in a community helps organizations determine where and how to use resources for the best return. Combined with the expert clinical models and actionable insights that the ACG System provides, ACG GeoHealth can help you deliver direct benefits to underserved communities or populations.

ACG GeoHealth delivers targeted insights so that you can develop the successful population health strategy you need. For more information on ACG GeoHealth, click here.
